Physician Services Agreement
OFFICIAL SIGNED RECORD · V2.2
Agreement Terms
Section 1. What is Locumize?
Locumize is a technology marketplace that connects licensed physicians directly with healthcare facilities seeking locum tenens coverage. We are not a staffing agency, employer, or healthcare provider. All work arrangements are negotiated and agreed upon directly between you and the facility — Locumize simply facilitates that connection.
Section 2. Your Role as an Independent Contractor
By using Locumize you are operating as an independent contractor — not an employee of Locumize. You decide which assignments to accept, set your own rates, and manage your own schedule. You are responsible for your own tax filings (including self-employment taxes) and compliance with state medical licensing requirements in every state where you practice.
Section 3. It's Free for Physicians — Always
Locumize charges physicians absolutely nothing. No sign-up fee. No placement fee. No commission deducted from your earnings. You negotiate your rate directly with the facility and receive 100% of that agreed amount. Locumize generates revenue exclusively from facility subscription fees.
Section 4. Liability Insurance
Professional liability (malpractice) coverage for each assignment is determined by agreement between the physician and the facility and specified in the assignment contract. Locumize does not require either party to carry coverage and does not provide malpractice insurance.
Section 5. Verification via Provider Trust
Verification is managed by Provider Trust — an independent third-party service. Locumize initiates the process on your behalf; you may be contacted to provide supporting documents.
Physician verification on Locumize is handled by Provider Trust, an independent third-party credentialing service. When you join the platform, Locumize will initiate a credentialing request with Provider Trust on your behalf. Provider Trust will verify your medical license, board certifications, education, work history, DEA registration, malpractice history, and OIG/SAM exclusion status. Credentialing must be completed before you can begin any assignment. Your credentialing status will be visible in your Locumize profile.
Section 6. Your Responsibilities
You agree to: (1) Keep your medical license current and in good standing in every state where you practice, and notify Locumize immediately if your license status changes. (2) Maintain an accurate, up-to-date profile. (3) Cooperate fully with the Provider Trust verification process. (4) Follow all applicable laws, regulations, and facility-specific policies during assignments. (5) Respond to facility inquiries in a timely, professional manner. (6) Submit timesheets accurately through the Locumize platform after each completed shift.
Section 7. Payments
Before an assignment begins, the facility pre-funds the full assignment value into a Stripe-managed escrow account. When you submit a timesheet and the facility approves it, Stripe automatically releases the escrowed funds to you — typically the same week. Locumize does not hold, access, or earn interest on escrowed funds; they are managed entirely by Stripe, and you keep 100% of your agreed rate. If a dispute is raised on submitted hours, escrow stays locked until it is resolved; Locumize support can assist by providing platform records but is not the payer or guarantor.
Where a facility includes non-hourly amounts on the job posting — such as housing or travel stipends or expense reimbursements — those amounts are captured with the assignment and released to you through Stripe escrow together with your approved hours, subject to any receipts or documentation the facility requires for reimbursements. Non-hourly amounts a facility does not include on the posting are not processed or guaranteed by Locumize.
Section 8. Background Check & Verification Consent
By accepting this agreement, you consent to Locumize (through Provider Trust) conducting: identity verification, medical license verification across all relevant state boards, DEA registration checks, board certification confirmation, OIG/SAM exclusion database searches, malpractice history review, and employment history verification. Verified credential information will be shared with facilities you engage with. You may dispute inaccurate results directly through Provider Trust's dispute process.
Section 9. Privacy & Data
Locumize collects your professional information to operate the platform matching process. When you express interest in a facility, your profile and credential summary will be shared with that facility. Please review our full Privacy Policy for details on how your data is handled. If you access patient health information during any assignment, you are solely responsible for full compliance with HIPAA and HITECH requirements.
Section 10. Limitation of Liability
Locumize provides the platform on an as-is basis and is not responsible for: the accuracy of information submitted by facilities, medical outcomes or clinical decisions during assignments, payment disputes between physicians and facilities, or employment-related issues arising from the physician-facility relationship. Because physicians use Locumize at no charge, Locumize's maximum liability to you is zero dollars.
Section 11. Termination
You may terminate this Agreement and close your account at any time. Locumize may suspend or terminate your account at any time, with or without prior notice, including if your medical license is revoked or suspended, if you misrepresent your credentials, if you breach these terms or our Terms of Service, for conduct that harms the platform or its users, or as required by law. We will notify you by email where reasonably possible, except where immediate action is required. Any obligations under active assignments remain in effect upon termination.
Section 12. Governing Law
This agreement is governed by the laws of the State of Texas. Any unresolved disputes will first go through a 30-day good-faith negotiation period, then to binding individual arbitration under the American Arbitration Association rules. Class action lawsuits are not permitted under this agreement.
